Text Box: In consideration of participation in the Extreme Tennis Academy for players under the age of 18, the parent/guardian and player: Hereby release and discharges the Extreme Tennis Academy, Inc., its officers, directors, employees, agents, and representatives )collectively called the realizes) from any and all action, claims and demands of every nature and character arising out of or resulting from: * Any damage, loss or injury suffered by the player on his/her property; or * Any damage, loss or injury suffered during or in any way connected with the player’s participation in any of the Extreme Tennis Academy programs or arranged by it to take place outside of it training premises; * Hereby give consent for your child to be taken on trips relating to tennis tournament, field trips and transportation in general; * Hereby waive any such action, claim or demand and covenant not to sue therefore.
